Repair a defective restoration or remove it and start from scratch? A habitual dilemma in most clinics.

When can it be repaired and when should it be replaced?
The University of Munich has recently determined 4 treatment options for this case:
- Control the restoration in case of minor defects, for example, discoloration of small marginal imperfections that are not harmful to the patient in case they are not treated.
- Renovation of the restoration if the defect can be solved without the need to add new material. For example, elimination of excess, polishing, bleached margins, sealing small cavities or pores.
- The repair is necessary if the restoration has small defects that require the addition of restorative material. Repair is required if imperfections can deteriorate if left untreated. For example, caries located adjacent to the margin of filling, fractures of the material of the restoration or adjacent pieces, marginal spaces.

What surfaces can iBOND Universal adhere to?
The new iBOND Universal is effective with zirconium, precious and non-precious metals, composites and compomers.
In addition it also achieves adhesion in fractured silicate ceramic whenever the first (silane) is applied iBOND Ceramic Primer before the adhesive to achieve greater impregnation and adhesion strength.
What advantages does this system offer in intraoral silicate and vitreous ceramic repairs?
In indirect restorations of silicate ceramic the surface of the ceramic needs to be etched with hydrofluoric acid, after which it is necessary to apply a silane, for example, iBOND Ceramic Primer.
The extraoral use of hydrofluoric acid to burn ceramics is safe. In contrast, intraoral use may involve the risk of severe necrosis of mucosal tissue or even bone necrosis. When using hydrofluoric acid in an intraoral repair on silicate ceramics, it is necessary that the rubber dam be well fixed, as well as that the acid be handled with extreme care.
Thanks to iBOND Universal, this risk is eliminated. Since the use of hydrofluoric acid is not required in the intraoral repairs of silicate ceramics. The use of iBOND Ceramic Primer, followed by the application of iBOND Universal on the surface of the ceramic, is sufficient.
What pre-treatment is required to prepare the tooth before the repair?
Grind the surface of the substrate to be repaired using a fine diamond bur or expose it to sandblasting. Gently clean and dry with air (oil-free). In the case of silicate / glass ceramic, the surface must first be prepared with iBOND Ceramic Primer.
Afterwards, apply iBOND Universal gently, rubbing for 20 seconds, dry and light cure for 10 seconds before applying the composite.
